Заключение диссертационного исследования на тему "Множественные синхронные эпителиальные опухоли толстой кишки: критерии диагностики, классификация и частота первично-множественных форм, пролиферативная активность"
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ
1. При морфологической верификации опухолей среди аденокарцином толстой кишки, кроме аденокарцином In situ и инва-зивных аденокарцином в тубулярно-ворсинчатых аденомах (М-8263/2, /3), выделять раки тубулярно-ворсинчатого строения, частота которых среди одиночных колоректальных опухолей составляет 10 7. К данным неоплазиям относить аденокарцино-мы, в которых тубулярный или ворсинчатый компоненты занимают от 20 до 80 % площади. Ввиду отсутствия такого гистологического типа опухоли в МКБ-0-2, для регистрации использовать новый цифровой морфологический код - М-8264/3.
2. Подсчет Ki-67-позитивных клеток в тех участках опухоли, где имеется их максимальное количество, позволяет определить "верхнюю границу" пролиферативного потенциала. Подсчет же клеток с экспрессией этого антигена в произвольно выбранных полях зрения может привести к недооценке пролифе-ративной активности новообразования. Все случаи, когда индекс метки Ki-67 при исследовании парафиновых срезов адено-карциномы толстой кишки очень низок (до 20 7.) или равен нулю. считать сомнительными для оценки пролиферативной активности опухоли, так как это может быть обусловлено как дефектами при предварительной обработке исследуемого материала, так и в иммуногистохимической технике.
3. При высоком индексе экспрессии антигена К1-67 (выше 65,4 X) в первично выявленной колоректальной аденокарциноме необходимо более тщательное обследование пациента на предмет наличия у него второго железистого рака в толстой кишке.
4. Наиболее частая встречаемость эпителиальных новообразований толстой кишки, как одиночных, так и множественных. после 50 лет, а также тот факт, что в 2-3 опухолях из 100 колоректальных аденом (а среди аденом ворсинчатого строения - в 8 % случаев) выявляются фокусы аденокарциномы. подчеркивают необходимость выполнения лицам, достигшим указанного возраста, тотальной фиброколоноскопии с биопсией всех выявленных новообразований.
