Переломным моментом в развитии трансплантологии па пути от первых экспериментов к широкой клинической практике можно считать открытие в конце пятидесятых годов прошлого века антигенов главного комплекса гистосовместимости. Существенно улучшился прогноз при включении в схему иммуносупрессии препарата нового поколения - циклоспорина А. Важным этапом стало применение в борьбе с острым отторжением поли-, а затем и моноклональных антител к лимфоцитам человека. Несмотря па успехи развития иммуносупрессивной терапии, острый криз отторжения все еще остается наиболее частой причиной гибели трансплантата. Ключевой проблемой становится быстрый выбор тактики лечения и определение продолжительности курса терапии. Биохимические параметры позволяют судить об эффективности антикризовой терапии не ранее, чем через 3-5 суток. Дифференциальная диагностика еще более затруднена при отсутствии первичной функции трансплантата.

Повторное использование антилимфоцитарных препаратов может быть ограничено образованием антител к ним в организме человека. Было показано, что количество наиболее серьезных инфекционных осложнений напрямую связано с дозой и продолжительностью применения антилимфоцитарных препаратов. В то же время уменьшение доз препаратов и сокращение курса лечения сопровождается снижением их эффекта. Поэтому одной из наиболее актуальных проблем является разработка критериев контроля эффективности антикризовой терапии, а также определение путей профилактики возможных осложнений.

Цель исследования

Определить оптимальную тактику применения препаратов поли- и моноклональных антител в лечении и профилактике острого отторжения после пересадки почки.

Задачи исследования.

1. Определить показания и противопоказания к применению препаратов поли- и моноклональных антител в лечении острого отторжения после пересадки почки.

2. Определить оптимальные сроки начала терапии препаратами поли- и моноклональных антител в лечении острого отторжения после пересадки почки.

3. Разработать критерии оценки эффективности антикризовой терапии у реципиентов после пересадки почки.

4. Провести многопараметровую сравнительную оценку применения разных схем антикризовой терапии у реципиентов после пересадки почки.

Научная новизна

1. Впервые проведена комплексная сравнительная оценка воздействия различных методов антикризовой терапии на экспрессию лимфоцитарных антигенов после пересадки почки.

2. Впервые проведен анализ результатов применения первого отечественного препарата моноклональных антител к CD3 рецепторам лимфоцитов человека «Атэма».

3. Разработан метод контроля за эффективностью антикризовой терапии с использованием многопараметрового цитофлоуметрического анализа.

Практическая значимость работы

В результате данного исследования разработана оптимальная тактика применения препаратов поли- и моноклональных антител к лимфоцитам человека в лечении и профилактике острого отторжения после пересадки почки, в том числе и первого отечественного препарата моноклональных антител к CD3 рецепторам лимфоцитов человека «Атэма».

Положения, выносимые на защиту

1. Включение антилимфоцитарных поликлональных анител в стартовую схему иммуносупрессии наиболее эффективно для профилактики острого отторжения после трансплантации почки.

2. Применение препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека наиболее эффективно в лечении острого отторжения после пересадки почки.

3. Применение препаратов поли- и моноклональных антител к лимфоцитам человека наиболее эффективно в течение первых трех суток после начала снижения функции трансплантата.

4. Препараты моноклональных антител к CD3 рецепторам лимфоцитов человека, созданные на основе ICO - 90 и антитела Muromonab- CD3 по избирательности связывания с популяциями лимфоцитов in vitro, по эффективности купирования реакции острого отторжения, а также по частоте развития побочных эффектов достоверно не различаются.

5. Для контроля эффективности антилимфоцитарных препаратов необходимо мониторирование экспрессии CD3, CD4, CD8, CD19, CD20, CD25 рецепторов лимфоцитов периферической крови.

6. Уменьшение дозы антилимфоцитарных препаратов по сравнению с расчетной ведет к снижению их эффектов при лечении и профилактике острого отторжения после пересадки почки.

Внедрение результатов работы

Разработанные схемы применения препаратов поли- и моноклональных антител к лимфоцитам человека используются при лечении реципиентов почечных трансплантатов в отделении оперативной нефрологии ФГУ «НИИ Урологии Росздрава» и в ГУЗ «Волгоградский областной уронефрологический центра».

 
 

Заключение диссертационного исследования на тему "Применение поли- и моноклональных антител к лимфоцитам человека в лечении и профилактике острого отторжения после пересадки почки"

Выводы.

1. В профилактике острого отторжения в раннем послеоперационном периоде после пересадки почки в качестве компонента стартовой иммуносупрессии более эффективно применение поликлональных антител по сравнению с моноклональными.

2. Раннее применение (в течение первых двух суток с момента начала острого криза отторжения) препаратов поли- и моноклональных антител к лимфоцитам человека позволяет существенно повысить их эффективность в отношение купирования острого криза отторжения независимо от предшествующей пульс-терапии кортикостероидами.

3. Критерием эффективности препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека можно считать снижение уровней CD3, CD4, CD8 положительных лимфоцитов периферической крови до 0,1- 1% при относительном росте CD 19+, CD20+ клеток через 3-6 часов после первого введения.

Критерием эффективности препаратов поликлональных антител можно считать снижение уровней CD3, CD4, CD8 положительных лимфоцитов периферической крови до 1- 5% через 12- 24 часа после первого введения.

4. Применение препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека при лечении острого отторжения позволяет снизить количество некупированных кризов отторжения в среднем па 14,3% и повысить годовую выживаемость трансплантатов на 12,0- 20,0% по сравнению с препаратами поликлональных антител.

При проведении комплексной сравнительной оценки препаратов моноклональных антител к CD3 антигенам лимфоцитов человека созданых на основе ICO - 90 и созданых на основе Muromonab- CD3 не выявлено достоверных различий по избирательности связывания с популяциями лимфоцитов in vitro и по эффективности купирования реакции острого отторжения.

5. Отсроченное включение в базовую четырехкомпонентную схему иммуносупрессии циклоспорина А позволяет существенно снизить нефротоксичность и, соответственно, период восстановления функции, а также увеличить клубочковую фильтрацию.

Практические рекомендации.

1. При низком уровне CD25 положительных лимфоцитов (менее 5%) в периферической крови для лечения и профилактики острого криза отторжения назначение препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека нецелесообразно, более эффективно применение поликлональных антител.

2. При отсутствии выраженного эффекта от пульс-терапии кортикостероидами в течении первых двух суток с появления клинических признаков острого отторжения показано применение препаратов поли- или моноклональных антител.

3. Продолжительность курса терапии препаратами поли- или моноклональных антител должна определяться возвращением к исходному уровню CD3+, CD4+, CD8+, CD 19+, CD20+, CD95+ лимфоцитов периферической крови.

4. При проведении курса антикризовой терапии препаратом моноклональных антител к CD3 рецепторам лимфоцитов человека, созданным на основе антител ICO 90, «Атэма» не требуется изменения расчетной дозы по сравнению с препаратами моноклональных антител, созданных на основе Muromonab- CD3.

5. При отсутствии немедленной функции трансплантата целесообразно отсроченное (по окончании курса профилактической терапии препаратами поликлональных антител) включение в протокол стартовой иммуносупрессии циклоспорина А.
