Актуальность темы

Неходжкинские злокачественные лимфомы (HX3JI) - это гетерогенная группа злокачественных лимфопролиферативных опухолей, различающихся по биологическим свойствам, морфологическому строению, клиническим проявлениям, ответу на терапию и прогнозу. В России они составляют 2,6% всех злокачествённых новообразований. В последние годы заболеваемость HX3JI имеет неизменную тенденцию к росту [Поддубная И. В., 2001, 2005; Jemal А., 2006]. Несмотря на улучшение результатов терапии, связанных с применением высокодозной программной полихимиотерапии, смертность от HX3JI увеличивается на 2% в год. Это определяет необходимость дальнейшего изучения механизмов злокачественной трансформации лимфоидных клеток и опухолевой прогрессии [Савченко В. Г., 2006; Воробьев А. И., 2002].

В настоящее время не вызывает сомнений, что процесс онкогенеза заключается в патологических изменениях вначале на молекулярном, а затем на клеточном уровне [Бочков Н. П., 2002], а предрасположенность к злокачественным новообразованиям и опухолевая прогрессия могут модифицироваться аллельными полиморфизмами генов [Thiagalingam S. А., 2006; Имя-нитов Е. Н., 2007].

В последние годы идентифицированы десятки полиморфных генов-кандидатов, которые могут принимать участие в формировании онкологического риска. Достаточно широко изучена роль полиморфных локусов dupl6bp 3 интрона, Arg72Pro 4 экзона и G—»А 6 интрона гена р53 [Zhang Z. W., 2003; Wu X., 2002], Arg399Gln гена XRCC1 [Casse С., 2003; Gao W. M., 2003; Hong Y. C., 2005] и del32bp гена CCR5 [Чердынцева H. В., 2007; Manes S., 1999; Murphy P. M., 2001] при солидных новообразованиях. Данные гены играют ключевую роль в таких процессах, как контроль клеточного цикла и запуск программированной клеточной смерти, репарация ДНК и хемотаксис, а их полиморфизмы могут изменять свойства кодируемых белков, вызывать нестабильность генома, что в ряде случаев способствует предрасположенности носителей этих генов к развитию опухолей различной локализации. Вместе с тем, роль данных полиморфизмов в патогенезе гемобластозов остается неизученной.

Ряд авторов указывают на то, что одни и те же аллели полиморфных ло-кусов генов р53, XRCC1 и CCR5 могут иметь как проонкогенный, так и защитный эффект, в зависимости от этнической принадлежности популяции [Koushik А., 2004], типа ткани, из которой развивается' опухоль [Tada М., 2001] и воздействия вредных факторов внешней среды [MurataM., 1998]. По этой причине данные литературы по вопросу о роли полиморфных вариантов ДНК в онкологическом риске и опухолевой' прогрессии часто являются противоречивыми, а результаты отдельных работ обладают плохой воспроизводимостью [Имянитов Е. Н., 2007].

Все это создает невозможность механического переноса результатов, полученных зарубежными авторами на солидных опухолях, на опухоли крови.

Вместе с тем, изучение роли полиморфных вариантов генов репарации ДЕК XRCG1 и запуска программированной'смерти р53 в патогенезе гемобластозов может быть важным, так как грубые изменения1 в них при НХЗЛ встречаются крайне редко [Duell Е. J., 2001; Marintchev А., 2000], и снижение их функциональной активности может быть связано с генетическими полиморфизмами. Полиморфизм гена хемокиновогорецептора CCR5, оказывающий влияние на плотность рецептора на поверхности клеточной мембраны, может изменять чувствительность лимфомных клеток к провоспалительным хемокинам и, таким образом, модифицировать метастатический потенциал опухоли.

Учитывая вышеизложенное, представляется* важным и перспективным исследование врожденных генетических полиморфизмов dupl6bp 3 интро-на, Arg72Pro 4 экзона и G—»А 6 интрона гена р53, Arg399Gln гена XRCC1 и del32bp гена CCR5 у больных неходжкинскими злокачественными лимфо-мами с целью установления их роли в предрасположенности к развитию заболевания и особенностях его клинического течения.

Цель работы: изучить полиморфизмы антионкогена р53, гена репарации ДНК XRCC1 и гена хемокинового рецептора CCR5 у больных неходжкинскими злокачественными лимфомами, и установить их связь.с риском развития заболевания и особенностями клинического течения.

Задачи исследования:

1. Изучить распределение частот аллелей и генотипов полиморфизмов dupl6bp 3 интрона, Arg72Pro 4 экзона и G—>А 6 интрона гена р53, Arg399Gln гена XRCC1 и del32bp гена CCR5 у больных неходжкинскими злокачественными лимфомами.

2. Сравнить распределение частот аллелей и генотипов данных полиморфных локусов при индолентных и агрессивных лимфомах.

3. Определить связь генотипов dupl6bp 3 интрона, Arg72Pro 4 экзона и G—>А 6 интрона гена р53, Arg399Gln< гена XRCC1 и del32bp гена' CCR5 с особенностями клинического течения заболевания.

4. Выявить ассоциацию изменений в исследуемых полиморфных локу-сах генов р53, XRCC1 и CCR5 с риском развития заболевания.

Научная новизна

Впервые исследована распространенность полиморфизмов dupl6bp 3 интрона, Arg72Pro 4 экзона и> G—6 интрона гена- р53, Arg399Gln гена XRCC1 и del32bp гена CCR5 у больных неходжкинскими.злокачественными лимфомами.

Впервые показана генетическая неоднородность пациентов1 с HX3JI по данным полиморфизмам, свидетельствующая о повышении частот минорных аллелей 4 экзона и сочетанных нарушений в,трех исследованных локу-сах антионкогена р53 при агрессивных^ вариантах заболевания, а также об увеличении доли пациентов с минорными аллелями интронных и экзонного полиморфизмов р53 и в гене XRCC1 при диффузной В-крупноклеточной лимфоме (ДВККЛ).

Получены данные о повышении частот минорных аллелей генов р53 и XRCC1, а также генотипов, их содержащих, у больных НХЗЛ высокой степени злокачественности по сравнению с популяционным контролем г. Новосибирска и НСО. Результаты свидетельствуют о различной степени предрасположенности индивидуумов популяции к развитию данных заболеваний.

Выявлено значительное повышение риска развития агрессивных вариантов лимфом у носителей Gln-аллеля гена XRCC1, а также ДВККЛ у носителей Gln-аллеля гена XRCC1, Рго-аллеля 4 экзона и m-аллеля 3 интрона р53 в сравнении с популяционным контролем.

На примере НХЗЛ подтверждена суммация эффекта Gln-аллеля XRCC1, заключающаяся в увеличении риска развития заболевания у Gln/Gln гомозигот относительно Arg/Arg лиц в большей степени, чем Arg/Gln гетерозигот.

Доказано, что в реализацию опухолевой прогрессии НХЗЛ CCR5 может быть вовлечен как рецептор провоспалительных хемокинов. Уменьшение уровня экспрессии рецептора на мембране клеток у носителей del-аллвля CCR5 может выступать благоприятным фактором, ограничивающим распространение опухоли по организму.

У пациентов с НХЗЛ показано неблагоприятное влияние на прогноз, согласно международному прогностическому индексу IPI, сочетанных нарушений в 72 кодоне 4 экзона гена р53 и 399 кодоне гена XRCC1, а также по трем полиморфным локусам р53.

Практическая значимость

Результаты, полученные при исследовании полиморфизмов dupl6bp 3 интрона, Arg72Pro 4 экзона и G—>А б интрона гена р53, Arg399Gln гена XRCC1 и del32bp гена CCR5 у больных неходжкинскими злокачественными лимфомами вносят вклад в современные представления о роли генетических изменений в патогенезе злокачественных заболеваний крови.

Данные о наличии минорных аллелей генов р53 и XRCC1, особенно при их сочетанном обнаружении, могут быть использованы в качестве дополнительных критериев при формировании групп повышенного риска по HX3JI, в частности, у лиц, имеющих длительный контакт с экзогенными канцерогенами, а также при оценке риска раннего прогрессирования заболевания.

Учитывая, что del32bp гена CCR5 может вносить существенный вклад в индивидуальную вариабельность метастазирования лимфомных клеток, исследование его генотипа у больных HX3JI может дать ценную информацию о течении опухолевого процесса.

Обнаружена связь сочетания минорных аллелей трех исследуемых ло-кусов р53, а также Pro-варианта 4 экзона р53 с Gin-вариантом гена XRCC1 с неблагоприятным прогнозом HX3JT, что может служить дополнительным критерием при планировании-курсов программной полихимиотерапии.

Положения, выносимые на защиту:

1. Частота встречаемости генетических полиморфизмов р53 и XRGC1 различна при индолентных и агрессивных лимфомах: при агрессивных вариантах заболевания отмечается снижение частоты диких аллелей и нормальных гомозиготных генотипов 3 интрона и. 4 экзона антионкогена р53, увеличение минорного Gln-аллеля и' гомозиготного Gln/Gln генотипа XRCC1, а также большая частота сочетанных нарушений по данным локусам в сравнении с больными индолентными лимфомами и здоровой популяцией.

2. Дикий тип гена хемокинового рецептора CCR5 коррелирует с большей распространенностью опухолевого процесса при HX3JT; сочетание минорных аллельных вариантов полиморфных локусов р53 и XRCC1 достоверно чаще регистрируется у больных лимфомами с неблагоприятным прогнозом.

3. Риск развития агрессивных лимфом у лиц, имеющих изменения в 399 кодоне гена XRCC1 возрастает от Arg/Gln гетерозиготных к Gln/Gln гомозиготным лицам.

 
 

Заключение диссертационного исследования на тему "Полиморфизм генов р53, XRCC1 и ССR5 у больных неходжкинскими злокачественными лимфомами"

ВЫВОДЫ

1. Распределение аллелей и генотипов по полиморфным локусам 3 интрона и 4 экзона гена р53, а также гена хемокинового рецептора CCR5 в группе пациентов с НХЗЛ соответствует популяционному.

2. У больных лимфомами частота дикого G-аллеля (87%), а также нормального G/G гомозиготного генотипа (78%) 6 интрона р53 достоверно (р<0,01) превышает таковые у здоровых лиц (78% и 63% соответственно).

3. У пациентов с НХЗЛ в сравнении со здоровыми лицами частота встречаемости нормального гомозиготного Arg/Arg генотипа XRCC1 в 1,5 раза ниже (22% против 34%, р<0,05), а минорного гомозиготного Gln/Gln генотипа в 1,9 раза выше (25% против 13%, р<0,05).

4. Агрессивные лимфомы характеризуются увеличением, в сравнении с индолентными лимфомами, доли лиц с Pro-вариантами 4 экзона р53 (64% против 45%, р<0,05), сочетанными нарушениями в 3 и 6 интроне (г=0,91 против г=0,89, р<0,001), 3 интроне и 4 экзоне (г=0,64 против г=0,30, р<0,001), а также 6 интроне и 4 экзоне (г=0,77 против г=0,38, р<0,001) гена р53.

5. У больных диффузной В-крупноклеточной лимфомой достоверно чаще (р<0,05) встречаются генотипы, несущие минорный аллель 3 интрона (50%), 4 экзона (79%) и 6 интрона (50%) гена р53, а доля Gln/Gln гомозиготных лиц по XRCC1 увеличена в 1,9 раза (43%) в сравнении с пациентами с диффузной В-мелкоклеточной лимфомой (14%, 44%, 18% и 22% соответственно).

6. Риск развития ДВККЛ, в сравнении с популяционным, значительно повышен у лиц, имеющих m-аллель 3 интрона (OR=2,6; р<0,05), Рго-аллель 4 экзона (OR=3,7; р<0,05) гена р53 и Gln-аллель гена XRCC1 (OR=6,5; р<0,05). Вероятность развития заболевания возрастает от Arg/Gln (OR=4,4; р<0,05) гетерозиготных к Gln/Gln гомозиготным лицам (OR=5,6; р<0,05) по гену XRCC1.

7. При диком типе гена хемокинового рецептора CCR5 отмечается большая распространенность опухоли при HX3JI, что проявляется в достоверном (р<0,05) увеличении числа больных с продвинутыми (III, IV) стадиями заболевания (87%), поражением селезенки (46%), костного мозга (74%), В-симптомами (88%), в сравнении с пациентами, имеющими del-аллель гена CCR5 (63%, 27%, 45% и 50% соответственно).

8. При сочетанных нарушениях в трех изучаемых локусах р53, а также в 4 экзоне данного антионкогена и 399 кодоне гена XRCC1 у пациентов с лимфомами отмечается повышение группы риска, согласно международному прогностическому индексу IPI.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Значительное повышение частоты Gln-аллеля XRCC1 у больных агрессивными вариантами неходжкинских лимфом, а также Gln-аллеля гена XRCC1, Рго-аллеля 4 экзона и m-аллеля 3 интрона гена р53 у пациентов с ДВККЛ, позволяет использовать их в качестве дополнительных критериев при формировании групп повышенного риска развития данных заболеваний, особенно у лиц, длительно контактирующих с вредными факторами внешней среды или с отягощенным семейным анамнезом.

2. Учитывая большую распространенность опухолевого процесса у больных НХЗЛ с диким генотипом CCR5, полиморфизм del32bp может быть использован для оценки вероятности раннего прогрессирования заболевания.

3. Полиморфизмы гена репарации ДНК XRCC1 и антионкогена р53 у пациентов с лимфомами целесообразно исследовать комплексно. Обнаружение сочетания минорных аллелей трех исследуемых локусов р53, а также Pro-варианта 4 экзона р53 с Gin-вариантом гена XRCC1, позволит патогенетически обоснованно выделить группу пациентов с плохим прогнозом и обсуждать возможность планирования у них более агрессивной программной терапии.
