В настоящее время благодаря усовершенствованию методов заместительной почечной терапии радикально изменился прогноз больных терминальной почечной недостаточностью. В то же время летальность больных ХПН остается высокой. По данным крупных многоцентровых исследований (НЕМО Study 2000, HDFR 1989, Cardiovascular Health Study 1998) основной причиной смерти больных уремией становятся болезни сердца и сосудов. В общей структуре смертности больных ТПН они составляют от 40 до 50% (Томилина Н.А. с соавт., 2003, U.S. Renal Data System 2000). При этом риск смертности от сердечно-сосудистых осложнений у больных ТПН, получающих лечение хроническим гемодиализом, в возрасте 45-54 лет превышает общепопуляционный в 65 раз (Zoccali С., 2002), а в возрасте 25-34 лет в 120-150 раз (Sarnak М. J., Levey A.S., 1999).

Высокая частота сердечно-сосудистых осложнений при ХПН дали основание рассматривать почечную недостаточность в качестве «васкулопатического состояния», которое под влиянием различных патогенетических механизмов вначале формируется как функциональное расстройство, а затем трансформируется в органическую сердечно-сосудистую патологию (Luke R.G., 1998). Наиболее частыми ее клиническими проявлениями являются атеросклероз сосудов и гипертрофия левого желудочка сердца (ГЛЖ).

Гипотеза об ускоренном развитии атеросклероза при ХПН была постулирована A. Linder еще в 1974 году, однако, до сих пор она далека до окончательного разрешения. По современным представлениям процессы ускоренного атерогенеза формируются при патологии почек задолго до начала лечения гемодиализом и лишь прогрессируют с момента начала заместительной почечной терапии (Смирнов А.В., 1999; Shoji Т., 2002).

По данным Foley R.N., (1995); Levin А. (1999), частота ГЛЖ у больных уже на додиализной стадии ХПН составляет 25% - 50%, что существенно превышает таковую в общей популяции, которая по данным Фрамингемского исследования составляет 17-20%. В настоящее время доказано, что ГЛЖ служит независимым и самостоятельным фактором риска сердечно-сосудистых осложнений, который через ряд метаболических, функциональных и структурных изменений повышает риск развития инфаркта миокарда, сердечной недостаточности, аритмии и внезапной смерти (Lorell ВН, 2000; Herzog А., 2002).

Вместе с тем, несмотря на констатацию высокой частоты сердечно-сосудистых осложнений при ХПН, остаются мало изученными факторы риска и механизмы развития патологии сердца и сосудов у пациентов с поражением почек. Практически отсутствуют данные о роли ремоделирования сосудов в развитии патологии сердца у больных уремией. Имеющиеся исследования выполнены в основном у пациентов, получающих заместительную почечную терапию (G. London с соавт. 1999-2005). Данных об изменении сосудов на ранних стадиях ХПН существенно меньше.

В последние годы благодаря внедрению в практику современных, надежных, неинвазивных методов исследования, в частности ультразвуковой допплерографии, появилась возможность изучать процессы ремоделирования сосудов и оценивать их роль в развитии патологии сердца. Изучение структурно - функциональных изменений сосудов и их связей с патологией сердца на начальных стадиях ХПН и в процессе прогрессирования почечной недостаточности позволит не только уточнить механизмы развития сердечно-сосудистых осложнений при уремии, но и выработать стратегию и тактику превентивных мер для уменьшения риска сердечно-сосудистых осложнений у больных ХПН.

Цель работы: выявить факторы риска сердечно-сосудистых осложнений у больных на додиализной стадии ХПН.

Задачи исследования:

1. У больных с додиализной стадией ХПН оценить частоту атеросклеротического поражения артерий и факторы риска его развития.

2. Определить частоту гипертрофии левого желудочка сердца и факторы риска ее развития у больных на додиализной стадии ХПН.

3. Оценить частоту острых сердечно-сосудистых осложнений у больных на додиализной стадии ХПН.

4. Выявить изменения структуры и функции общих сонных и бедренных артерий у больных на додиализной стадии ХПН: оценить толщину комплекса интима-медия (ТИМ), наличие атеросклеротических бляшек, растяжимость сосудов.

5. Выявить факторы, влияющие на структурно-функциональные изменения артерий.

Научная новизна

Изучена распространенность атеросклеротического поражения сосудистых бассейнов на додиализной стадии ХПН. Установлено, что на додиализной стадии ХПН развитие атеросклероза определяет сочетанное воздействие «традиционных» (возраст, пол, АГ, избыточный вес, гиперлипидемия, курение, отягощенная наследственность по ССЗ) и «почечных» (показатели фосфорно-кальциевого обмена) факторов риска.

Впервые показано, что при ХПН атеросклероз сосудов развивается на ранних стадиях и длительно протекает бессимптомно. Снижение СКФ и повышение креатинина крови являются самостоятельными, независимыми факторами риска развития доклинической стадии атеросклероза. При снижении функции почек происходит постепенная смена влияния факторов риска с «традиционных» на «почечные».

Оценены частота выявления гипертрофии левого желудочка и острых сердечно-сосудистых осложнений на додиализной стадии ХПН.

Впервые установлена зависимость между структурно-функциональными изменениями магистральных сосудов шеи и нижних конечностей и ГЛЖ. Впервые установлено развитие ремоделирования сосудов на додиализной стадии хронической почечной недостаточности. Выявлены факторы, связанные с заболеваниями почек, которые способствуют структурно-функциональным изменениям сосудов при почечной недостаточности (артериальная гипертония, гиперкреатининемия, снижение скорости клубочковой фильтрации, гиперлипидемия).

Практическая значимость

Выявлены «традиционные» и «почечные» факторы риска атеросклероза, включая доклиническую его стадию, у больных на додиализной стадии почечной недостаточности. • Представлены современные методы диагностики бессимптомной, доклинической стадии атеросклероза.

Определены факторы риска гипертрофии левого желудочка сердца (ГЛЖ) - доклинической стадии сердечной патологии - и острых сердечно-сосудистых осложнений.

Уточнена роль ремоделирования артерий в развитии сердечнососудистых осложнений.

Изучение факторов риска и механизмов развития сердечно -сосудистых осложнений позволит выработать стратегию и тактику коррекции процессов ремоделирования сердца и сосудов у^ больных на ранних стадиях хронической почечной недостаточности и поможет снизить риск развития сердечно-сосудистых осложнений у этой категории больных.

выводы

1. У больных на додиализной стадии хронической» почечной-недостаточности (ХПН) выявляется высокая, частота сердечнососудистых осложнений, в основе которых лежит ремоделирование магистральных артерий и миокарда и ускоренное развитие атеросклероза сосудов.

2. Развитие сердечно-сосудистых, осложнений, (атеросклероз сосудов, гипертрофия левого желудочка сердца, острые сердечнососудистые осложнения) на додиализной. стадии ХПН* связано с сочетанным воздействием «традиционных» (пол, возраст, АГ, гиперлипидемия, избыточный вес, курение, отягощенная наследственность по ССЗ) и специфичных для ХПН (нарушение фосфорно-кальциевого обмена, снижение СКФ, гиперкреатининемия) факторов риска. По мере прогрессирования ХПН' воздействие «почечных» факторов1 риска усиливается; в то время как «традиционных» - снижается.

3. При заболеваниях, почек атеросклероз сосудов развивается на ранних, стадиях* ХПН и длительно протекает бессимптомно, проявляясь увеличением толщины, комплекса интима-медии (ТИМ) артерий: По данным ретроспективного исследования клинические проявления атеросклероза аорты, сосудов-сердца, головного мозга, почек, нижних конечностей отмечались, в 37%, 24%, 18%, 14%, 5% случаев соотв. Частота выявления «доклинической» стадии атеросклероза-почти в 3 раза превышала частоту его клинических проявлений.

Снижение СКФ и повышение креатинина крови является самостоятельным, независимым фактором развития доклинической стадии атеросклероза.

4. На консервативной стадии ХПН развивается гипертрофия миокарда (37,3%) и нарушается геометрия сердца. Концентрическое ремоделирование отмечено в 31,3%, концентрическая гипертрофия - в 19,3%, эксцентрическая гипертрофия - в 18,1%. По мере прогрессирования ХПН частота ГЛЖ и эксцентрическая модель ГЛЖ возрастала. Между структурно-функциональными изменениями сосудов и наличием ГЛЖ отмечается тесная связь.

5. При заболеваниях почек структурно - функциональные изменения магистральных сосудов (увеличение ТИМ - в 65% в ОСА и 68% - в ОБА; кальциноз сосудистой стенки - 11% в ОСА и 27% в ОБА и образование атеросклеротических бляшек - 22% в ОСА и 24% в ОБА) развиваются на додиализной стадии ХПН.

Практические рекомендации

1. На додиализной стадии ХПН существует высокий риск развития сердечно-сосудистых осложнений, который обусловлен сочетанным воздействием как «традиционных» (пол, возраст, АГ, избыточный вес, гиперлипидемия, курение, наследственность), так и «почечных» (снижение СКФ, гиперкреатинемия, нарушение фосфорно-кальциевого обмена) факторов риска. Изучение механизмов их воздействия является основой для разработки стратегии и тактики лечения и профилактики этих осложнений.

2. При заболеваниях почек атеросклероз сосудов развивается на ранних стадиях ХПН и длительно протекает бессимптомно. Ультразвуковая допплерография позволяет оценить структурно-функциональное состояние сосудов. Увеличение ТИМ артерий может рассматриваться как объективный диагностический показатель для раннего выявления атероскперотического процесса. Существует связь между структурно-функциональными изменениями крупных сосудов и наличием ГЛЖ.

3. У пациентов, страдающих хронической почечной недостаточностью, на додиализной стадии возникает ремоделирование магистральных сосудов шеи и нижних конечностей. Оценка толщины комплекса интима - медия и определение растяжимости сосудистой стенки - важные инструменты для стратификации сердечно - сосудистого риска у этих больных. В связи с высоким сосудистым риском у данной категории пациентов, необходима ранняя коррекции факторов, вызывающих ремоделирование сердечно-сосудистой системы.
